## A dated report on one contractor

For a bid, a subcontract or an audit: proof of what the state registry showed for one contractor on a given date, compiled as a PDF.

2. Click **Dated registration report**: the form below fills itself in. You can also type the name or
the certificate number here.
3. We compile the report by hand and send it to you as a PDF by e-mail, with a card payment link for $19.

### A dated report on this entity

The pages of this site are free and will stay free: an official fact belongs to no one. What is sold is the **compilation work** — what the public registers say about this entity on a given date, and what they do not say.

#### What the report contains

- the certificate: number, legal and trade name, issue and expiration dates, status
- the answers on record: employer registration, workers' compensation, apprenticeship, wage assessments, debarment, final determinations
- **what was not found**, written down: the checks made and the empty results
- the exact citation: state file and date

#### What the report is NOT

It is not a document of the Department of Labor and not the certificate itself. It states what the state file showed on its date; it does not replace the check a contractor must make in the state's own registry.

This is an order form. The request reaches us and we confirm it by e-mail before any payment. Nothing is charged at this stage.
